This tutorial assumes you have a working knowledge of PSP.  I used PSPX2

Elements = El  Copy/Paste = C/P  Duplicate = Dup  Paper = P  Template = Temp

Elements - Please reference my example for placement

New Image 700 X 700

C/P El 11  Resize 85%

Select Magic Wand - Right click inside frame

Selections - Modify - Expand 10

C/P P 12  Resize 60%

Selections - Invert - Delete

Arrange - Move paper down

C/P Large Tube  Resize 75% and position

Hit Delete - Move down

Selections - Select None

Add a Drop Shadow to tube  Settings:  4/4/60/10/Black

Add a Drop Shadow to Frame  Setting:  3/3/60/3/Black

C/P  El 77  Behind Frame

C/P El 56  Dup x 3  Move one to left corner, 

the other 3  up and down along the right side

C/P El 10  Resize 80%  Free Rotate Left 20

C/P El 23

C/P El 87  Resize 85%  Free Rotate Left 20

C/P El 29  Resize 70%  Dup Resize 90% Dup

C/P El 18  Dup x 2  Mirror 1 move left

C/P El 49  Resize 65%

C/P El 47  Resize 85%

Add Drop Shadow to all the Elements

Settings : 2/2/40/2/Black

 C/P Tube  Resize 65%

Drop Shadow Settings:  3/3/60/2/Black

Layers - Merge - Merge Visible  

Crop - Resize - Sharpen if preferred

Now add your Artist Copyright and your License info. and Name
